I agree by asking the University of New Haven Office of Information Technology (OIT) to repair my personal computer and/or device that I will not hold them responsible for any damage to my device(s) or loss of data to my equipment. I accept all responsibility to back up my data and understand that loss of data is a potential risk by the service I am requesting.

OIT provides hardware and software repairs at its discretion; however, screen repairs are not offered. I understand that OIT does not guarantee that my device can be successfully repaired or restored to full functionality. Additionally, OIT is not responsible for any pre-existing issues, including but not limited to malware, viruses, or software corruption, that may impact the performance of my device.

If I request student tech support to back up my data on my behalf, I understand that the data will be moved back to my device, not copied, and all data will be permanently deleted from OIT’s storage upon transfer. No copies or backups will be retained by OIT.

I agree to pick up my equipment within 60 days of notification, and I understand that if I do not collect my equipment within this time frame, OIT may dispose of my equipment and accessories. OIT is not responsible for any data or accessories left with the device after this period. My equipment may only be returned to me, and I must provide the documentation OIT requests, such as a photo ID, at the time of pickup.

I acknowledge that I am responsible for providing valid licenses for any software that needs to be reinstalled on my device. Any pirated or illegal software, including but not limited to unauthorized Windows license keys or Microsoft Office installations, will be removed by OIT, and OIT will not be able to reinstall or restore them.

By submitting this form, emailing, having OIT fill out this form on my behalf, and/or dropping off my device for repair, I acknowledge that I have read, understand, and agree to the terms outlined in this waiver.
